Sterilization, Manohasa's order that doubled the concession bill

On March 16, 2017, the former director general of the Mandatory Health Insurance Fund, Erion Manohasa issued an order to the Hospital Service Directorate and the Saniservice company on how the Sterilization concession contract would be implemented.

In the letter published by Oligarkia.al, Manohasa orders textually: "The Saniservice concessionary company will start providing the sterilization service for steam and low-temperature packages at QSUT, initially only in the Maxillofacial Surgery Service, Vascular and in the Plastic Surgery Service.”

Although at first glance this looks like just a routine bureaucratic document, in reality this is an order that has charged a financial bill of millions of euros at the expense of Albanian citizens because it limited free packaging to only three surgical wards.

mECHANISM

The sterilization concession contract provided for 5 types of services. They were the sterilized instruments for interventions with high complexity that had a price of 37.7 thousand Lek per package, sterilization for interventions with medium complexity at a price of 22.3 thousand Lek and sterilization for surgical interventions with low complexity, for which the concessionaire company charged a price of 13.9 thousand lek without VAT.

In addition to these three classes, there were two more for very minimal interventions. These were the steam packs, for which the company charged 266 ALL and the low temperature packs, which were paid 1090 ALL per pack.

Manohasa's order limited the use of the last two classes to only 3 wards of QSUT. Consequently, all minimal interventions were performed using the low complexity class, the one that was charged 13.9 thousand lek per package, even though they could be performed with simple packages.

So instead of paying 266 ALL per package, 13.9 thousand ALL was paid per package because doctors were forbidden to use steam and low temperature packages by Manohasa's order.

Numbers

Në vitin 2017, numri i ndërhyrjeve me kompleksitet të ulët, për të cilat kompanisë koncesionare i paguhej 13.9 mijë lekë pa TVSH ishte 24.5 mijë ndërhyrje. Në vitin 2018, shifra kërceu në 55.3 mijë ndërhyrje.

Të detyruar nga fakti që nuk mund të përdornin paketimet e thjeshta me avull dhe ato me temperaturë të ulët, mjekët përdornin instrumentat e kompleksit të ulët. Kompanisë koncesionare i interesonte kjo situatë sepse arkëtonte më shumë para. Ndërsa qytetarët shqiptarë, në vend të ujit paguanin shampanjë. Në vend të 266 lekëve për ndërhyrje, paguanin 13.9 mijë lekë për ndërhyrje.

Kjo ishte arsyeja se pse në vitin 2018, fatura e paguar nga buxheti për koncesionin e sterilizimit kërcëu në 1.7 miliardë lekë ose gati 16 milionë euro. Kjo marrëzi zgjati nga fillimi i vitit 2017 deri në gusht të vitit 2018, kur drejtori i ri i FSDKSH Eduart Ahmeti hoqi kufizimin dhe urdhëroi përdorimin e paketimeve të thjeshta në të gjitha repartet kirurgjikale të QSUT dhe ato të spitaleve të tjera të vendit.

Menjeherë pas urdhrit të Ahmetit, numri i ndërhyrjeve me kompleksitet të ulët nisi të zbriste me shpejtësi dhe në vitin 2019 ai ra në 25 mijë nga 55.3 mijë që kishte qenë një vit më parë. Me pak fjalë 2.14 herë më pak se në 2018-ën. Por dëmi ishte bërë sakaq dhe ishte masiv.

Milionat që u tretën

Vetëm si pasojë e ndalimit të përdorimit të paketimeve të thjeshta, qytetarët shqiptarë vlerësohet se kanë paguar të paktën 7 milionë euro padrejtësisht për kompaninë koncesionare.

Në atë kohë FSDKSH e justifikoi urdhrin me faktin se nuk mund të kontrollonte dot numrin e paketimeve të thjeshta dhe për të ndalur abuzimin me shifrat e tyre ndaloi përdorimin. Pra për të ndalur abuzimin me setet që kushtonin 266 lekë copa, Fondi ndaloi përdorimin e tyre dhe paguante setet me 13.9 mijë lekë copa. Një marrëzi dhe abuzim kolosal, i cili i shtohet listës së abuzimeve në shëndetësi.

Therefore, the sterilization concession should not only be investigated on how the contract was made, but also how it was implemented.
